Case Manager

Location: Project Rehab Community Alternatives Program (Grand Rapids)

Summary

This position is responsible for case management services, assessments, treatment planning, individual counseling, communication skills (oral and written), group lecture and didactic, facilitating groups, maintaining and ensuring all paperwork is completed routinely, timely and accurately.

Qualifications

Bachelors degree in Social Work, Psychology, or Human Services field required. CAC-M preferred. Knowledge of group dynamics and sensitivity to the needs of clients. Must be a skilled communicator with highly developed written and verbal skills.
